- Clip original product label with the model and serial number (Motorola phones) or PIN number (BlackBerry Curve 8350i smartphone) from the side of the product package. Do not submit a photocopy of the original.
- Make a photocopy of the original purchase receipt detailing the date, price paid and product purchased.
