Follow these steps to receive your rebate:

1. For your convenience, faster payment and real time tracking, register online at: http://southernlinc.4myrebate.com using offer code SLW-10000. Or download and complete the mail-in rebate form (please do not do both)
2. Clip original product label with the model and serial number (Motorola phones) or PIN number (BlackBerry Curve 8350i smartphone) from the side of the product package. Do not submit a photocopy of the original.
3. Make a photocopy of the original purchase receipt detailing the date, price paid and product purchased.
4. Send Mail-in Rebate form or Online Rebate form, original product label and copy of purchase receipt to:
