How to Get your Rebate
Submit all of the following to receive your rebate: (NOTE: Rebate forms must be postmarked no later December 31, 2008 and received by January 16, 2009.)
- Download and complete the Mail-in Rebate form.
- Clip out original product label with serial number from the bottom of the product package. Do not submit a photocopy of the original.
- Make a photocopy of the original purchase receipt detailing the date and product purchased.
- Send Mail-in Rebate form, original product label and copy of purchase receipt to:
